If you would like a copy of your medical records, please complete a release form that may be located on the link below. If you are requesting a paper copy, there will be a $25 processing fee. DTC will always send medical records to another physician at your request free of charge after receiving a completed release form.

If you are a current DTC patient, you may call our office or send us a secure message to schedule your appointment.

 

Please note:

If it has been more than 3 years since your last visit, you will be considered a new patient and will need to complete new patient paperwork at your next appointment. You will, however, continue your care with the same physician you have historically seen.

Follow-up appointments include a 10-minute grace period for arrival. If you arrive more than 10 minutes after your scheduled time, you will be asked to reschedule.

PxP Portal makes it easy to stay connected to your care. Through the portal, you can:

View your lab results

Request prescription refills

Communicate directly with your provider

Access messages, medication lists, and continuity of care documents

If you have not yet enrolled, please call our office to obtain your enrollment token number. Once your portal is activated, you will be responsible for updating your demographic information directly in the portal. Our office can assist with password or token resets.

 

For medication refills, please continue to request them first through your pharmacy. The portal can be used to follow up on those requests, view updates from your provider, and ask non-urgent questions. If you encounter any issues with the portal, please contact our office and we will be happy to help you get reconnected. PxP Portal.

  • If you need a medication refill, please contact your pharmacy so that a request will be generated through the electronic medication system. This is the quickest way to get a refill completed. 

  • If you do not have your refill at the pharmacy after 24-48 hours, please contact the office at 817-263-0007 to ensure that we received the pharmacy’s request. If we did not receive the pharmacy’s request, we will be able to enter a request for refill at this time. 

  • If your refill is denied, you will need to schedule an appointment prior to being issued a medication refill. 

  • If your medication is no longer covered by your insurance, please contact your insurance to find a comparable medication that is in their formulary prior to contacting DTC, so that your provider may consider this new medication. 

  • If you discover that your medication requires a prior authorization, please find first if there is an approved comparable. If there is no comparable medication and a prior authorization must be completed, please contact DTC to notify us of your need for a prior authorization. Please be aware that a prior authorization approval from your insurance may take more than one week to receive.

  • DTC no longer provides patients with samples routinely. Instead, samples are limited to new medication starts and at the time of appointment. 

  • If you are unable to afford your medication, please contact your insurance to find out what a more affordable comparable medication may be. Once you have received this information, please utilize NextGen PxP Portal and let the medical receptionist know of your medication expense concern and the possible comparable drug so that we may see if your physician is comfortable changing your medication.

  • In the event that you receive a lab bill that seems very high, please contact the lab company first. If the lab company indicates that your lab was denied due to medical necessity or medical coding, please utilize NextMd with the specific test denied, the invoice number and the date of service. Your provider’s medical assistant may be able to submit additional coding to support the lab. In the event that specific labs are not covered services with your insurance, DTC will not be able to submit coding to cover the lab. It is important for you to be aware of your lab coverage and discuss any of these concerns with your provider prior to receiving the labs.

     

    Additionally, certain insurances only cover a certain number of tests per year. This coverage applies to all providers across your care. Please ensure that you do not receive any labs outside of your coverage limits unless you are comfortable with the lab costs associated as DTC will not be able to credit or remove lab charges once they are received by a patient as these lab services are not billed by DTC.

LAB SERVICES

LABORATORY ORDERS

At DTC, lab orders are entered electronically by your physician, usually during your current visit in preparation for your next appointment. We ask that you complete your labs about one week before your scheduled appointment.

Fasting: Fasting is generally required only for the lipid panel, unless your provider specifies otherwise.

If no lab order is found: If you arrive at the lab and no order has been entered, please call 817-263-0007. We will submit a stat request for your provider to enter the order while you wait. Please note that, because providers are often in clinic, this may take up to 30 minutes.

Paper orders: If you prefer, we can provide a printed lab order at checkout for your next appointment.

You can also request a lab order ahead of time through our PxP portal.

LAB RESULTS

Your lab results are typically available to your physician within 48 hours to one week after completion, depending on the specific tests performed. Once received, your physician will review the results and provide care recommendations as needed. For this reason, the usual turnaround time for results from DTC is about one week.

 

Please note: You may see your lab values sooner through a lab app on your phone. However, this does not mean your physician has reviewed them yet. If it has been more than 10 business days since your labs were drawn and you have not received results or follow-up from your physician, please send us a message through PxP Portal to check on the status.
