We are excited that you would like to become a patient at DTC! If you have never been seen by a DTC physician, please ask your primary care doctor or referring physician to send us a referral. Once we receive your referral and baseline medical information, our team will contact you to schedule your appointment at the most appropriate time.
Please note: this is a physician-to-physician referral (not an insurance referral) and is required by DTC regardless of your insurance plan. If you have previously been seen by a DTC physician, you will continue your care with the same physician you have historically seen, as DTC does not transfer patients between physicians within the practice.

At DTC, we do our best to keep your appointment on time. To help us serve you efficiently, please be sure your new patient paperwork is completed before your visit.
Our New Patient Packet can be completed online through Jotform (link below). We strongly encourage you to fill this out in advance.
If you prefer to complete your paperwork at DTC, please arrive 30 minutes before your scheduled appointment so you can be checked in on time.
Please note: If you arrive late, or arrive on time without your paperwork completed, you may be asked to reschedule.
